⚠️ Hospitals Are Increasingly Requiring Doula Insurance

Many hospital systems and birthing centers now require a certificate of insurance from doulas before granting access to labor and delivery areas. This trend is growing. Getting covered now protects your access to clinical birth settings — and protects you if a client blames you for any aspect of their birth experience.

Doula Insurance: What You Need

🤱

Professional Liability

Most Important
$25–$40/mo

Covers claims your support, advice, or actions during birth or postpartum care caused harm. Covers your legal defense even for frivolous claims — common in birth-related litigation. Required by many hospital L&D units.

🛡️

General Liability

Recommended
$15–$30/mo

Covers third-party bodily injury and property damage. If you meet clients in your home office, their home, or at events, GL covers slip-and-fall and property damage claims.

🌿

Placenta Encapsulation Add-On

If Additional Services
Rider: $10–$25/mo

Placenta encapsulation carries food safety liability separate from doula professional liability. Verify your policy includes or excludes this service — many standard doula policies exclude it.

🎓

Childbirth Education Coverage

Educators
Included or add-on

If you also teach childbirth classes, verify your policy covers educational services separately from hands-on doula support. Some policies require an endorsement for classroom instruction.

Doula Insurance FAQ

Frequently Asked Questions

What insurance does a doula need?+
Doulas need: (1) Professional liability — covers claims that your support, advice, or presence during birth or postpartum care caused harm to the client or baby. Doulas are not medical providers, but they can still face lawsuits alleging they interfered with medical decisions, provided negligent advice, or failed to act appropriately during an emergency. (2) General liability — covers third-party bodily injury and property damage. If you meet clients in your home office or theirs, GL covers slip-and-fall and property damage claims. Professional liability and GL together run $35–$65/month for most doulas.
Does DONA International require doula insurance?+
DONA International strongly recommends professional liability insurance for all certified doulas but does not currently mandate it for DONA certification. However, many hospitals, birthing centers, and medical practices that allow doulas on-site are increasingly requiring proof of GL and professional liability insurance. Some states and hospital systems have begun requiring COI (certificates of insurance) before granting doulas access to labor and delivery areas. CAPPA (Childbirth and Postpartum Professional Association) similarly recommends but does not mandate coverage.
How much does doula insurance cost in 2026?+
Doula professional liability is affordable: Professional liability only ($1M): $25–$40/month. GL + professional liability ($1M each): $40–$65/month. Annual premium: $500–$800/year for both. Birth doulas face somewhat higher professional liability exposure than postpartum doulas due to the clinical setting and medical complexity of birth. Doulas serving high-risk pregnancies or working with multiple midwifery or OB practices should consider higher limits ($2M aggregate).
Can a doula be sued even though they are not a medical provider?+
Yes. Doulas have faced lawsuits in situations including: allegedly encouraging a client to refuse medical intervention that later proved necessary; failing to summon emergency services quickly enough; providing postpartum advice that a client claims worsened a complication; emotional distress claims from dissatisfied clients. The fact that doulas are not medical providers does not protect them from professional liability claims. Professional liability insurance covers your legal defense costs even for frivolous claims — which are common in birth-related litigation.
Does doula insurance cover postpartum services?+
Yes — most doula professional liability policies cover both birth doula services and postpartum doula services under a single policy. If you offer additional services (childbirth education, lactation support, placenta encapsulation), verify these are included in your policy or require a rider. Placenta encapsulation carries separate food safety liability and may require a separate product liability endorsement. Lactation consultant services may also require a separate LC professional liability policy if you hold IBCLC credentials.
What is the best doula insurance carrier in 2026?+
For birth and postpartum doulas: DONA International affiliated programs, Ancient Art Midwifery Institute, and specialty carriers that understand the birth industry are best. ERGO Next and Hiscox offer instant online quotes for professional liability + GL. For doulas also offering childbirth education: Allied Healthcare Professionals Insurance or Healthcare Providers Service Organization (HPSO) offer broader coverage for multiple birth-related services. Shop multiple carriers — doula insurance rates vary significantly by carrier.
